APP is proud to partner with Jordan Healthcare Center to offer prioritized access to psychotherapy services for their clients. This relationship will allow us to offer all current APP (seen at any location) access to primary care services which would include psychiatric medication management, substance use treatment, and educational/support groups. The new APP Southwick team will have in-person therapy space within the Jordan Healthcare location and also be offering telehealth sessions.
FOR JHC CLIENTS: How it Works
Click on a therapists profile, then click the button labeled “Become a client at Advance Psychotherapy Practice”, click “Join the Waitlist”. Indicate “Jordan Healthcare Practice” and the therapist you are requesting.
APP will verify your insurance details and your therapist will reach out to you to begin the process (electronically signing consent docs, etc.) as well as discuss your mental health needs.
APP will bill your insurance directly and copays and deductibles (if applicable) will be automatically and securely charged through our electronic healthcare system.

About Jordan Healthcare Center
Jordan Healthcare Center is a primary care practice in Southwick, Massachusetts, led by Jennifer Jordan, DNP, APRN, FNP-C. As an experienced nurse practitioner, Dr. Jordan practices advanced nursing, focusing on the patient, not their symptoms.
Dr. Jordan received her Doctor of Nursing Practice (DNP) at Elms College in Chicopee, Massachusetts, and is a board-certified family nurse practitioner.
Jordan Healthcare Center sees patients ages 12 and up, providing adolescent care, gynecology care, and men’s health services. Dr. Jordan also specializes in chronic disease management and has an addiction recovery program.
To better serve its diverse community, Jordan Healthcare Center has Spanish-speaking staff and offers translation services for any other languages.
